We may collect personal information from various stakeholders such as donors, beneficiaries, partners and service providers that are necessary for the implementation of PSFl's programs and services.
The personal Information we collect may include, but is not limited to:
1. First 2 letters of Mother’s Name.
2. First 2 Letters of Father’s Name.
3. Birth Order.
4. Birthdate.
5. Sex at Birth.
6. Self-Identity.
7. Mobile Number.
8. Email Address.
9. Preferred Delivery Address.
10. Number of Sex Partners.
and others as indicated in the Registration Form.We may collect your personal information thru forms (application forms, surveys, attendance log sheets, etc.), email and data from program partners.

When you inquire via PSFI's email address posted on the PSFI website, additional information may be requested from you in responding to your queries.
How do we use your personal information?
We collect, use and process your personal information for the following purposes but still in compliance with the provisions of data privacy law:
- Provision of services - To deliver the services requested by our clients, including the scheduling of appointments and the processing of service request.
- Customer support - To respond to inquiries and provide customer support related to the services provided by our organization.
- Re-Testing and Adherence Reminders - To notification and reminders to clients who requested a specific HIV Care Service, including appointment confirmation, request reminders, refill request, request cancellation, and other information that are related to a client’s requests.
- Data analysis - To perform data analysis and research to improve the services provided by our partners, stakeholders and enhance the overall customer experience.
- Legal compliance - To comply with applicable laws and regulations, including the Philippines Data Privacy Act (RA 10173).
We have implemented appropriate security measures to protect our clients' personal information from unauthorized access, disclosure, alteration, or destruction.
Information collected may also be used to comply with the requirements of regulatory agencies in relation to doing business with our stakeholders and in implementing programs.
With whom do we share your personal information?
We may need to disclose your personal information on a strict need to know basis to our principal funder/donor, service provider, program partner organizations, and competent regulatory and statutory agencies as required by applicable laws related to abovementioned purposes.
In some cases, we may hold your data for a longer period of time after the conclusion of the program implementation and contractual obligations: 1) for statistical purposes but we retain only the anonymized data; and 2) if there is a legal or regulatory reason to do so.
